Owner/Landlord Information and Request Forms

As a landlord, the Utilities want you to have control on how your property is billed when vacant. We are providing you with several options on how you want to provide the information to us.

To connect a tenant we have provided the following options to request this service:
To complete the on-line form click HERE. To print the form click HERE. Click HERE for the Word document. Once completed, either fax it to 812-376-2427, drop it off at the main service center or, if you completed the Word document, just attach it to an email back to us.

To disconnect a tenant we have provided the same options:
To complete the on-line form click HERE. To print the form click HERE. Click HERE for the Word document. Please complete and return the form by one of the methods listed above.

We have also provided on-line Landlord Service/Notification Agreement forms. If you would like to initially complete, update, or change this form, authorizing the Utilities to automatically place services in your name when service is discontinued by your tenant, click HERE. If you want to be notified before tenant services are put in your name click HERE. You also have the option to print the form by clicking HERE, or you can click HERE for the Word document format. If you choose either of these options, please complete and return the form by one of the methods listed above.
