Landlord Service Notification Agreement

Upon notification I, the Landlord, authorize Columbus City Utilities to place utility services in my name for the properties listed below.

I further understand that as the property owner I will be responsible for any unpaid sewer balance that is left unpaid by my tenant.

This agreement shall remain in full force and effect for all properties owned by me until it is either canceled in writing or superseded by a new agreement.
