Landlord Service Agreement

By completing the below form I, the Landlord, authorize Columbus City Utilities to automatically place utility services in my name, when service is disconnected by the tenant, for the properties listed.

I further understand that as the property owner I will be responsible for any unpaid sewer balance that is left unpaid by my tenant.

This agreement shall remain in full force and effect for all properties owned by me until it is either canceled in writing or superseded by a new agreement.
